Informed Consent and Waiver/Release of Liability:

As the parent or legal guardian of the child listed on this application, I agree and understand that swimming can be a hazardous activity and that there are risks inherent in the sport of swimming. I also recognize the fact that no one can drown-proof my child and that there is no substitution for eye-to-eye contact when my child is around water. I agree to allow my child to participate in Desert Swim school swim lessons and hereby agree to indemnify and hold harmless Waterworks On Wheels, Inc. DBA Desert Swim School, its coaches, instructors, coordinators, directors, and additional employees, against any liability resulting from any injury that may occur to my child while participating in Desert Swim School swim lessons. I also agree to indemnify Desert Swim School for any damages incurred arising from any claims, demand, action or cause of action by my child. Desert Swim School assumes no responsibility for any personal property placed in or about the facility where swim lessons are conducted.

Payment Policy: 

Desert Swim School clients enjoy the benefit of NO contracts and NO additional fees.

Cancellation Policy:

If you cancel your scheduled (backyard or outdoor) lessons within 72 hours (before lessons begin) of your scheduled session date, the balance of your payment can be transferred as a credit for a future session minus a $50 processing fee. (Emergencies will be evaluated on a case by case basis)

There are no hidden costs at Desert Swim School

Make up Policy:

We do not have scheduled make ups. If you are in a group lesson, and are unable to make the lesson, please let us know at your earliest convenience/12-24 hour notice. Make ups will be handled to the best of our ability on a case by case basis. Due to the fact that ratios are 4:1 max, and abilities must be similar, it must be a case by case basis. 

If you are in a backyard lesson, this lesson will be added to the end of your scheduled dates.

*Every swimmer in the backyard class must miss the lesson

Photo/Video Release: 

I understand that photos and/or video are occasionally taken at Desert Swim School and that any photo and/or video taken of my child may be used for Desert Swim School publicity purposes.
